UDTC Instructor Training Questionnaire

Our first session for instructor training will be June 16th from 7-9. The second session will be June 29th from 7-9.  I have suggested 4 - 2 hour sessions minimum. At this point these are the only ones scheduled. Instruction will include VHS, DVD, lecture and actual instructing.
This questionnaire is for me to get to know you.   It is to learn of your personal experience and get to know where you are as a trainer. It will allow me to assess your knowledge base and factor this with the response from others in class. Having this information ahead of time will allow me move forward for everyone in the first class. 
Please be as detailed as you would like to be. Please write ‘I don’t know’ or 'I have no experience' where applicable. When I have received and reviewed all the questionnaires I will be able to set up a class modeled specifically for each of you and together as a group.
